China’s Gen Z starts medical aesthetics earlier

A generation raised on skincare routines, ingredient explainers and before-and-after videos is approaching medical aesthetics differently from its parents. Rather than waiting for deep wrinkles or volume loss, younger Chinese consumers are entering clinics earlier for skin boosters, collagen treatments, energy-based procedures and low-dose injectables intended to preserve their appearance. This preventive mindset is changing both the treatment mix and the commercial logic of China’s medical aesthetics industry. Consumers aged 21 to 30 accounted for 56.9% of first-time users in a 2024 KPMG survey, while the accounting firm forecasts the wider market to grow from 311.5 billion RMB ($46 billion) in 2023 to 1.3 trillion RMB ($192 billion) by 2030, a compound annual growth rate of 22.6%. “The traditional consumer often entered medical aesthetics to correct a visible problem. Younger consumers increasingly enter before there is a major problem to correct,” Carol Zhou, senior vice president and head of medical beauty and business development at Shiseido, told Jing Daily. “That changes the business from correction to management.” Prevention turns procedures into long-term skin management For an older generation, medical aesthetics often began with something that needed fixing: pronounced wrinkles, lost facial volume or a feature the consumer wanted to reshape. Younger patients are arriving before those concerns become obvious, looking instead at hydration, collagen levels, pigmentation and the first signs of structural ageing. Many begin with relatively affordable, low-downtime treatments such as skin boosters, water-light injections or photorejuvenation. As their needs change, clinics can move them towards collagen stimulators, botulinum toxin and protocols combining injectables with lasers or radiofrequency. “Gen Z increasingly sees medical aesthetics as part of their regular self-care routine rather than something to do only when signs of aging appear,” said Fei Xu, market intelligence and consumer insight director at Asia Cosme Lab. “Instead of waiting to correct problems, they start early with smaller, non-invasive treatments and maintain them over time, much like a skincare routine.” China’s light medical aesthetics market grew from 50.2 billion RMB ($7.4 billion) in 2018 to 146 billion RMB ($21.5 billion) in 2023. The number of consumers rose from 7.4 million to 23.5 million during the same period, supported by lower prices, shorter recovery times and wider access through digital platforms. That cadence has more in common with skincare replenishment than traditional cosmetic surgery. Hwee Chung, China beauty insight director at Worldpanel, describes prevention as an FMCG-like model (fast-moving consumer goods): smaller individual transactions, but more frequent and continuous consumption. “Lifetime value is more about the length of the relationship than the size of the unit transaction,” Chung said. Subtle treatments that cause minimal disruption can create a relationship lasting “20 to 30-plus years,” she added, provided the clinic can retain the customer. South Korea opened the market, China rebuilt the model South Korea helped normalize the idea that aesthetic treatments could be light, affordable and routine, rather than dramatic interventions reserved for a small, affluent group. Its influence remains visible in China’s focus on skin quality, low-downtime procedures and combination protocols bringing together lasers, skin boosters and injectables. Chinese consumers still travel to Korea for its clinic efficiency, competitive prices and treatments built around ingredients such as PDRN. Visitors from mainland China represented 22.3% of South Korea’s international medical patients in 2024. Dermatology accounted for 71% of the services they received, compared with 11% for plastic surgery. But the domestic industry has moved beyond importing Korean treatments and beauty templates. “Korea’s model made light medical aesthetics known to more Chinese consumers, but China is creating its own playbook on the back of a robust digital ecosystem in both information search and commerce,” said Chung. So-Young made procedures, clinic information and prices easier to compare. Meituan placed treatments within a familiar local-services marketplace. Xiaohongshu (also known as RedNote) connected medical terminology with skincare education and peer reviews, allowing a new ingredient or technique to move from clinical discussion into mainstream beauty conversation within weeks. Melissa Gan, an independent beauty and wellness researcher and former Euromonitor analyst, sees vertical integration as a defining difference. So-Young operates 33 clinics, manufactures its own filler through a supplier partnership and uses its booking platform to drive demand and collect consumer data. “That combination of clinic ownership, product manufacturing, and platform data is a Chinese innovation,” Gan said. “Korea has superb clinic-level efficiency, but China is building a full-stack beauty system that connects products, services, and digital engagement in a way Korea hasn’t replicated at scale.” The consumer’s journey is also less confined by industry categories. She may alternate between topical skincare, beauty salons, home devices and medical procedures without viewing them as separate markets. For brands, that makes the opportunity broader, but the competitive set harder to define. Xiaohongshu moves consultation before the clinic Young consumers now arrive at clinics with a vocabulary once held mainly by doctors and product companies. They compare different collagen types, read about injection depths and recovery times, check medical qualifications and study before-and-after photographs before booking an appointment. Xiaohongshu has more than 300 million monthly active users, with 90% of its content generated by users. About 72% consider the platform’s content reliable, according to KPMG’s report. Searches for photorejuvenation, hyaluronic acid and laser freckle removal point to its role in introducing consumers to minimally invasive procedures. “The biggest shift is that the consultation no longer begins in the clinic,” Zhou said. “By the time a young Chinese consumer meets a doctor, she may already have spent weeks researching technologies, comparing products, watching recovery diaries and reading peer reviews.” Xu said patients who once asked, “What should I do?” are now more likely to arrive asking whether they should choose treatment A or treatment B. That does not make the physician less important. As treatment plans involve more products and technologies, doctors must judge whether the procedure trending online is safe, necessary or suitable for the consumer’s facial anatomy and skin condition. In some cases, the consultation becomes an exercise in reversing expectations already formed online. Gan describes Xiaohongshu as a feedback loop rather than a retention engine. It surfaces anxieties, desired outcomes and signals of trust, but it cannot guarantee clinical satisfaction. “Platform-derived trust may drive initial clinic selection, but it does not guarantee retention, repeat visits, or long-term loyalty,” she said. Clinics still have to turn online expectations into consistent results, transparent communication and personalized follow-up care. Collagen moves beyond China’s standardized influencer face The treatments gaining momentum also point to a change in what consumers want from medical aesthetics. An earlier era was associated with the “influencer face” (网红脸): a high nose bridge, pronounced eyelids, narrow jaw and pointed chin designed around a recognizable template. Younger consumers now talk about the “mother-born face,” (妈生脸), or which preserves individual features while making the person look fresher or more refined. “The goal is now to look like a fresher, better version of yourself, not a different person,” Xu said. This helps explain why collagen injectables, regenerative materials and combination treatments are taking a larger place beside Botox and conventional hyaluronic acid fillers. The focus is moving from filling a wrinkle or adding volume towards skin density, elasticity, firmness and longer-term tissue condition. China’s collagen retail market expanded from 20.47 billion RMB ($3 billion) in 2020 to 58.42 billion RMB ($8.6 billion) in 2023, a CAGR of 41.8%. Frost & Sullivan projects it will reach 269.5 billion RMB ($39.8 billion) by 2030. Recombinant collagen grew by 64.5% annually between 2020 and 2023, compared with 28.5% for animal-derived collagen. Domestic biotechnology companies including Giant Biogene, Jinbo Bio, Jland Biotech and Everon Healthcare are developing different collagen types, production systems and Class III medical-device pipelines. China’s competitive position is shifting from lower-cost alternatives to imported fillers towards homegrown biomaterials, patents and integrated treatment ecosystems. Zhou cautions against presenting the trend as a simple embrace of natural beauty. “Chinese consumers remain highly demanding about results,” she said. “What has changed is the definition of a good result.” The desired outcome should be noticeable to the patient without being easily identifiable to everyone else. It also requires a more complex understanding of aging. Skin quality, pigmentation, collagen loss, muscle movement and facial structure cannot be addressed by a single product. “The future belongs less to one hero procedure and more to the right protocol for the right person at the right time,” Zhou said. Falling prices complicate Gen Z’s lifetime-value promise A consumer entering the category in her 20s should, in theory, be more valuable than someone booking one corrective procedure later in life. China’s injectable medical aesthetics market grew from 25.7 billion RMB ($3.8 billion) in 2018 to 67 billion RMB ($9.9 billion) in 2023 and is forecast to reach 147 billion RMB ($21.7 billion) by 2027. But the longer relationship is forming as prices fall. A surge of new approvals has added products across hyaluronic acid, collagen and regenerative injectables. Some treatments now have similar ingredients, indications and operating methods, giving platforms and clinics little room to compete beyond price. So-Young has offered chemical peels for 149 RMB ($22) and skin boosters for 399 RMB ($59), according to Gan. Prices for some regenerative injectables have fallen from manufacturer guidance above 16,000 RMB ($2,370) to platform offers of 4,999 RMB ($740), 2,999 RMB ($445) and, during group-buying promotions, 999 RMB ($150). “The earlier entry is real, but the lifetime value story is now complicated by falling prices,” Gan said. “At those prices, even a 10-year relationship may not generate the same revenue as one corrective procedure used to.” A consumer may receive a skin booster every one or two months, generating more appointments and a richer history of her changing skin. The real asset may be the data created across those visits, allowing clinics to personalize protocols and introduce skincare, maintenance products or more advanced treatments later. Yet Gen Z is unlikely to remain with the same provider merely because she entered early. “A younger consumer may have a much longer relationship with medical aesthetics, but she is also more informed, more experimental and potentially less loyal,” Zhou said. “The real opportunity is not a younger patient. It is a longer relationship — but that relationship will be much harder to own.” Trust, not traffic, will decide the winners China’s current patient journey is fragmented. A consumer can discover a collagen treatment on Xiaohongshu, compare its price on Meituan, choose a clinic through reviews, receive the injection from a doctor, buy post-treatment skincare elsewhere and return to social media for recovery advice. Each part of the industry may optimize its own transaction, while the consumer is left to connect the entire journey. For Chung, commoditization makes doctor and clinic expertise more important, not less. The strongest providers will act as aesthetic consultants, giving precise and personalized recommendations without overwhelming patients with products they do not need. “Amid the commoditization of medical aesthetics, the real edge will come from two things,” Chung said. “The first is medical expertise capable of building trust; the second is an experience recognizing that treatments require ‘emotional engagement and satisfaction,’ not merely a hospital-style appointment.” Product innovation will remain critical as domestic collagen and regenerative technologies advance. Digital platforms will still control discovery. But repeat business will depend on whether clinics can join diagnosis, treatment, recovery, maintenance and prevention into one coherent service. Zhou calls the missing advantage continuity: understanding what the consumer’s skin needs now, what it may need next and when no intervention is necessary. “The next stage of medical aesthetics will not be won by whoever sells the most treatments,” she said. “It will be won by whoever earns the right to guide the consumer over time.”
